Brian Robert Thompson was an American healthcare executive who served as the Chief Executive Officer of UnitedHealthcare, the insurance division of UnitedHealth Group, from April 2021 until his death in December 2024. At the time of his death, he was one of the most influential figures in the American health insurance industry, overseeing coverage for tens of millions of people across the United States.
Although Thompson generally maintained a low public profile compared to many Fortune 500 executives, his death brought national attention to both his career and the healthcare giant he led.

Early Life and Education
Thompson was born on July 10, 1974, in Ames, Iowa, and grew up in the nearby community of Jewell Junction. He excelled academically from a young age and graduated as valedictorian from South Hamilton High School in 1993.
He attended the University of Iowa, where he earned a Bachelor of Business Administration degree in accounting in 1997. Once again, he graduated at the top of his class, receiving special honors and the highest distinction. Friends and classmates later described him as highly intelligent, hardworking, and driven.
Building a Career at UnitedHealth
After college, Thompson began his career at PricewaterhouseCoopers (PwC), working in transaction advisory services. In 2004, he joined UnitedHealth Group, a move that would shape the rest of his professional life.
Over the next two decades, he held a series of senior leadership positions involving Medicare, Medicaid, employer-sponsored insurance, and financial operations. Colleagues viewed him as a skilled executive with deep knowledge of healthcare finance and government health programs.
In 2021, Thompson was promoted to CEO of UnitedHealthcare, the largest division of UnitedHealth Group. Under his leadership, UnitedHealthcare remained the largest health insurer in the United States, and company profits increased substantially during his tenure.
Leadership and Controversies
As CEO, Thompson oversaw a business that insured more than 50 million Americans and employed over 100,000 people. He promoted initiatives focused on “value-based care,” a healthcare model intended to reward providers for patient outcomes rather than the volume of services delivered.
However, UnitedHealthcare also faced criticism during his leadership. Consumer advocates, healthcare providers, and some lawmakers questioned the company’s claim-denial practices and other insurance policies. In addition, lawsuits and regulatory scrutiny involving the company and some executives attracted public attention. UnitedHealthcare denied wrongdoing and defended its business practices.
The Fatal Shooting in New York
On the morning of December 4, 2024, Thompson was in New York City to attend a UnitedHealthcare investor conference. As he walked outside the Hilton Hotel in Midtown Manhattan, he was shot in what authorities described as a targeted attack. He was 50 years old.
The killing sparked a nationwide manhunt and became one of the most heavily covered criminal investigations in the United States. Authorities later arrested Luigi Mangione and charged him in connection with the shooting. Mangione has pleaded not guilty and remains presumed innocent unless proven guilty in court.
Family and Legacy
Thompson is survived by his wife, Paulette, and their two sons. Friends, colleagues, and former business associates described him as intelligent, approachable, and deeply committed to both his family and his work. Several former colleagues noted his ability to connect with people at all levels of the organization despite leading one of the largest healthcare companies in the country.
His death became more than a story about a corporate executive. It also ignited a broader national discussion about healthcare, insurance companies, executive security, and public frustration with the American healthcare system. Even as the criminal case continues, Brian Thompson remains a significant figure in the history of UnitedHealthcare and the wider healthcare industry.
